Accu-Chek® + 360CareTM: Your Health Coach makes diabetes self-management easy. Individuals will have access to a health coaching team which includes a registered dietitian and a registered nurse trained in diabetes education. The program features a convenient app that is an available hub of support, encouragement, & accountability. With it, users will be able to track and update their health numbers, schedule appointments, check on progress towards goals, get nudges and encouragement, set-up medication reminders, and more.
